After Computer is turned on screen reads system test failed, is this a software or hardware problem, or both, any suggestions? |

Sounds like hardware...

A couple of [quick] suggestions:
1. Make sure that all of your cables have no bent or broken pins and are plugged in securely.
2. Enter the ROM monitor after you get the 'test failed' message [hold down both the command keys / and the tilde key]. When you get there - type the letter p ... and make sure that show verbose boot is yes. Also try changing the SCSI tests to no [if it's set at yes]. Then power down - and restart.
Good luck! |

I found this NeXT error code chart while searching:
4x FPU Test
5x SCC Channel Test
6x SCSI Channel Test
7x Ethernet Channel Test
8x MO Drive ECC Test
9x Real Time Clock
ax Two Bit Graphics Test
cx System Timer Test
dx Event Counter Test
ex Sound Out Test
It also looks like your hard drive is dead.
There is a lot of good info in this link and it's searchable:
